Finasteride (Propecia) Guide for Hair Loss

The complete guide to finasteride for male pattern baldness—how it works, effectiveness, side effects, and what men should know before starting.

What is Finasteride?

Finasteride (brand name Propecia) is an FDA-approved prescription medication for male pattern baldness. It's a pill taken once daily that blocks DHT, the hormone responsible for hair follicle miniaturization in genetically susceptible men. It's one of only two FDA-approved treatments for male hair loss.

How Finasteride Works

Finasteride inhibits 5-alpha reductase, the enzyme that converts testosterone to DHT (dihydrotestosterone). By reducing DHT levels by about 70%, it slows or stops the miniaturization of hair follicles. This prevents further hair loss and, in many cases, allows follicles to recover and produce thicker hair.

Effectiveness

Clinical studies show finasteride stops hair loss in about 90% of men who take it, and about 65% experience some regrowth. Results are best in men who start treatment early, before significant hair loss has occurred. It's most effective for crown and mid-scalp areas, with less impact on the hairline.

Key Points

  • Start as early as possible for best results
  • Takes 6-12 months to see full effects
  • Must continue taking it to maintain results
  • Works better combined with minoxidil

Side Effects and Concerns

The most discussed side effects are sexual—decreased libido, erectile dysfunction, and reduced ejaculate volume. These occur in about 2-4% of men and typically resolve after stopping the medication. There's ongoing debate about 'post-finasteride syndrome,' though large studies haven't confirmed persistent effects after discontinuation.

Key Points

  • Side effects are uncommon (2-4%)
  • Most side effects resolve if you stop
  • Lower doses (0.5mg) may reduce side effects
  • Discuss concerns with your doctor

How to Take Finasteride

Take 1mg once daily, with or without food. Consistency is key—take it at the same time each day. If you miss a dose, skip it and continue with your regular schedule. Don't take extra to make up for missed doses. Some men take 1mg every other day or 0.5mg daily with similar effectiveness.

Finasteride vs. Dutasteride

Dutasteride (Avodart) blocks more DHT than finasteride (90% vs 70%) and may be more effective for hair loss. However, it's not FDA-approved for hair loss, has a longer half-life, and may have more side effects. Some doctors prescribe it off-label for men who don't respond to finasteride.
